SAFETY FLASH: CARGO COLLISION DURING BACKLOADING OPERATION

INCIDENT OVERVIEW

During a backloading operation between two vessels, a cargo unit was being lowered onto the deck of the receiving vessel. After signalling the crane operator to lower the hoist and disconnect the stinger, the banksman prepared for final placement. At that moment, the receiving vessel experienced sudden rolling prompting the crane operator to adjust the winch to stabilise the main block.

As the winch was lifted again to compensate for vessel movement, the cargo swung unexpectedly and made contact with other cargo already stowed on deck, becoming temporarily jammed. The situation was quickly brought under control, and no personnel injuries occurred.

POSITIVE OUTCOMES

  • The crane operator maintained control of the load and avoided releasing the block, preventing uncontrolled cargo movement.
  • The crew recognised the situation early and responded promptly to prevent escalation.
  • Clear banksman signals were used throughout the operation.
  • The cargo was handled in line with the pre-agreed stowage plan.

ISSUES IDENTIFIED

  • Vessel rolling was underestimated, resulting in unexpected cargo instability.
  • The deck of the receiving vessel had limited clearance, increasing collision risk.
  • The lifted load lacked sufficient stabilisation during the adjustment, allowing uncontrolled swinging.
  • A short communication delay occurred between the crane operator and banksman during the rolling event.

ROOT CAUSE

The incident was primarily caused by unexpected vessel rolling during cargo placement. The crane operator’s subsequent hoist adjustment to counter the movement resulted in the suspended cargo swinging in close proximity to other deck items, leading to the collision.

KEY TAKEAWAYS AND ACTIONS

  • Conduct thorough toolbox talks before lifting operations to highlight the potential effects of vessel motion.
  • Incorporate vessel movement considerations into all lifting risk assessments.
  • Ensure clear continuous communication between crane operators, banksmen and deck crew when handling dynamic loads.
  • Be prepared to exercise Stop Work Authority if stability concerns arise.
